2) The Sub District and the County

3) The Entry Number is the Register

4) The Date the Birth occurred and the place of birth.

5) The name of the child (if any)

6) Whether Male or Female

7) The father's name

8) The Mother's name and any previous surnames she has had.

9) The father's occupation.

10) The name, address and relationship to the child of the person who registered the birth
